In order to help you find your perfect pet, we ask that you complete this form. Please answer all questions completely and honestly. The information you provide will help us match you with the most compatible pet to fit your home and lifestyle. If you are approved to adopt one of our animals, this form will become part of your adoption contract. In order to be considered as an adopter, you must:

  • Be at least 18 years old.
  • Have a valid driver's license and identification showing your current address.
  • Be able to verify that pets are allowed where you live, if renting or otherwise no the property owner.
  • You must be physically, financially and emotionally able to care for an animal.
  • Allow us to visit your home prior to the adoption and at least once before the end of a three to four week trial period. We reserve the right to approve or deny adoption as we see fit.

    1. You understand that this animal is to be adopted as a pet and is not to be kept kenneled or chained outside.

    2. You understand that there is a four week trial period, during which we or you may decide that the adoption will not work out and that the animal must be returned to to Barking Mutt.

    3. You understand that if you ever must give up this dog, it must be returned to Barking Mutt Animal Welfare Society and will not be turned in to any other shelter, rescue or individual without our express written permission.

    4. You agree to allow a representative of Barking Mutt to inspect the home and yard environment, before the adoption process begins and at least once again before the trial period is ended.

    5. You understand we cannot guarantee our pets are housebroken.

    6. You understand that although we will tell you everything we know about the pet, we cannot guarantee temperament and/or behavior.

    7. You understand that if the information provided on this application is found to be untrue/incorrect, the adoption contract will be deemed null and void and You agree to allow an agent of Barking Mutt Animal Welfare Society to remove the animal from the premises occupied by you, and entry shall not constitute trespass.
